About the Support Navigator Form

The Support Navigator Form is a tool designed to help connect students to the right campus support services. Submissions notify the student’s assigned HomeBase and initiate proactive outreach and follow-up from a Support Specialist.

In its current phase of implementation, Instructional faculty and Admissions & Records (A&R) staff are able to submit referrals.

This tool supports students who may benefit from help navigating college processes, referrals to services, or connection to a supportive campus community.

Purpose of the Form

The Support Navigator Form helps students:

  • Connect with their HomeBase support team
  • Receive referrals to services such as counseling, basic needs support, or tutoring
  • Learn about campus resources and opportunities
  • Build community and belonging at ARC

The goal is to ensure students receive timely outreach and support, especially those who may not self-advocate or know where to begin.

Before Submitting: Please Notify the Student

When a referral is submitted, the student will automatically receive an email notification letting them know:

  • A referral was submitted
  • Who submitted it
  • That someone from their HomeBase will be reaching out

When to Use This Form

Use this form when a student:

  • Are struggling with attendance, participation, coursework, or appear overwhelmed
  • Are unsure where to go for help or need support navigating college processes (enrollment, financial aid, forms, etc.)
  • Could be connected to leadership or involvement opportunities (clubs, student government, campus programs)
  • Express barriers related to basic needs or technology access
  • Would benefit from connection to campus resources, programs, student life, or a supportive check-in
  • Are new to ARC or still building confidence, routine, or familiarity with available support

Students will typically receive outreach within 3–5 business days.

When Not to Use This Form

This form is not intended for:

Situation

Classroom management issues or behavior concerns

Concerns about a student’s mental health, safety, or well-being
