How to request for keys and access card

Keys and access cards are requested through Work Orders.
Work Orders are submitted by each area's Administrative Assistant, Clerk, or Dean.

Access Card

  • Submit a Work Order for a new access card, additional area access, or replacement card
  • Provide the following Information; (Last, First name), Employee ID#, Title (Faculty, Staff, Administrator)
  • Clarify the type of access required: area access, toggle capability and/or alarm access
  • An email will be sent by Operations to the intended recipient requesting a photo in .jpg format
  • Once these steps are completed, another email is sent out by Operations that the card has been printed and ready for pick-up

Note: Please allow up to one week to receive new cards.

Keys

  • Note: It is highly recommended that each department submit a Work Order to request keys at least 3 weeks prior to semester start. If an area does not have new faculty information they can submit the anticipated number of new faculty for the key request.
  • When a Work Order is submitted, Operations looks to see if we have an available key on-hand that can be distributed. If Operations doesn’t have a key in stock, the Work Order will be transferred to Facilities Management (FM) to have the key(s) made.
  • Once the key has been cut by Facilities Management (FM) and received by Operations, an email will be sent out by Operations to the intended recipient indicating it’s ready for pick-up.
  • Deans/Supervisors need to ensure employees turn in keys to Campus Operations when assignments change or when employees resign

Report a lost key

Use this form to report a lost key to Campus Operations. The loss of a key must be reported immediately and signed in as lost (must come in to Operations office). The lost key is the responsibility of the key holder; any replacement cost will be borne by the key holder’s program, department, or campus.
